Guidelines for Patient Referral
The Vascular Laboratory assists in the diagnosis and follow-up of patients with the following diseases and disorders:
- Cerebrovascular disorders
TIA, CVA, vertebrobasilar insufficiency
- Lower extremity ischemia
Mild claudication to threat of limb loss
- Venous Disorders
Deep venous thrombosis, chronic venous insufficiency
- Thoracic Outlet Syndrome and Upper extremity ischemia
- Aneurysms of the aorta
- Visceral ischemia
- Complications related to vascular repair
Recurrent thrombosis of vascular repair, recurrent stenosis
- Digital ischemia
vasospastic disorders, embolic events, small vessel disease
For Patient Referral
A Vascular Lab Request form must be filled out by the referring physician prior to scheduling this test. The test desired and the indication for the test must be completed, as well as the physician's pager number, phone number, and fax number.
The completed form is then faxed to (310)794-9603. A staff member from our center will then contact the patient for scheduling an appointment.
Note: Please call (310)206-6294, option 2 to schedule urgent studies, but please fax the requisition beforehand. This number may also be called to facilitate test scheduling if the patient is still in the physician's office.
